Understanding CalAIM

What Is CalAIM?

CalAIM — California Advancing and Innovating Medi-Cal — is a multi-year initiative by the California Department of Health Care Services (DHCS) to improve the quality of life and health outcomes for the millions of Californians covered by Medi-Cal, especially those with the most complex needs.

CalAIM recognizes that health is shaped not just by clinical care, but by the social and environmental conditions of everyday life — housing, food, safety, transportation, and social connection. By addressing these factors alongside medical care, CalAIM aims to reduce hospitalizations, prevent nursing home placements, and help people live safely and independently in their communities.

For eligible Medi-Cal members, CalAIM offers three major programs: Enhanced Care Management (ECM), California Integrated Care Management (CICM), and Community Supports. Senior Home Advocates provides all three — directly, in person, at no cost to qualifying members.

Whole-Person Care

CalAIM addresses clinical, behavioral, functional, and social needs together — not in separate silos. Every care plan considers the full picture of a person's life.

Community-Based Delivery

Services are delivered in the community — at home, at the hospital, or wherever the member is — not through a call center or portal. In-person engagement is required.

Person-Centered Approach

Members drive their own care plans. Goals, priorities, and services are built around what matters most to each individual — not a one-size-fits-all protocol.

Equity and Access

CalAIM is designed to reach the most vulnerable and underserved Medi-Cal members — including seniors, people experiencing homelessness, and those with complex behavioral health needs.
