Enhanced Care Management (ECM) is a statewide Medi-Cal Managed Care Plan benefit that provides equitable, person-centered, community-based care management for members with the most complex health and social needs. It is part of California's CalAIM initiative — a multi-year effort to transform and improve Medi-Cal for the state's most vulnerable residents.
At the heart of ECM is a single Lead Care Manager — a dedicated professional who serves as your primary point of contact, coordinates all of your clinical, behavioral, and social services, and advocates for your needs across every provider and system you interact with.

The 6 Core ECM Service Components
1. Outreach & Engagement
In-person outreach to identify and engage eligible members, including those who are hard to reach or have disengaged from care.
2. Comprehensive Assessment & Care Planning
In-depth assessment of clinical, behavioral, functional, and social needs — resulting in a personalized, member-driven care plan.
3. Care Coordination
Coordination across all providers, plans, and community organizations — ensuring nothing falls through the cracks.
4. Health Promotion
Education, coaching, and support to help members understand their conditions, manage their health, and prevent crises.
5. Transitional Care
Support during transitions between care settings — hospital to home, SNF to community — to prevent readmissions and ensure continuity.
6. Member & Family Support
Education and support for family members and caregivers, plus coordination with and referral to community and social support services.
Who Qualifies for ECM?
ECM is available to Medi-Cal Managed Care members across 9 defined Populations of Focus. SHA specializes in seniors and older adults — but serves members across multiple populations.
Experiencing Homelessness
Currently unhoused or at imminent risk of losing housing.
At Risk of Chronic Homelessness
History of homelessness with complex medical or social needs.
Transitioning from Incarceration
Recently released from jail, prison, or youth detention.
High Utilizers of Emergency Services
Frequent ER visits or hospitalizations that could be prevented with better care coordination.

Leaving a skilled nursing facility, IMD, or other long-term care setting.
