What Is California Integrated Care Management?
California Integrated Care Management (CICM) is a statewide Medi-Cal Managed Care Plan benefit specifically designed for dual-eligible members — individuals who qualify for both Medicare and Medi-Cal. CICM provides comprehensive, person-centered care management that integrates both programs into a single, coordinated care experience.
As part of California's CalAIM initiative, CICM represents the evolution of care coordination for the state's most complex and vulnerable seniors. Beginning in 2026, "ECM-like" services for dual-eligible members are transitioning to CICM, which layers California-specific requirements on top of federal Dual Eligible Special Needs Plan (D-SNP) standards.

Who Is Eligible for CICM?
CICM is designed for dual-eligible members who have complex needs and can benefit from integrated, coordinated care. You may qualify if you meet one or more of the following criteria.
Older Adults with Complex Needs
Seniors managing multiple chronic conditions, functional limitations, or cognitive decline who need help coordinating care across multiple providers.
Recent Hospital or SNF Discharge
Members recently discharged from a hospital, skilled nursing facility, or other institutional setting who need support transitioning safely back to the community.
High Utilizers of Emergency Services
Members with frequent emergency room visits or hospitalizations who would benefit from proactive care coordination to prevent future crises.
Serious Mental Illness or Behavioral Health Needs
